Why this Project Was Needed:
The benefits of Medication Reconciliation (Med Rec) have been outlined in published studies showing a reduction in the number of clinically significant errors or omissions related to medications. As a result, Accreditation Canada includes Med Rec as part of all Accreditation Standards for hospital inpatients and outpatients.
Med Rec will also help reduce readmission rates as the entire care team has access to a single reference list of all the medications that our patients are taking.
Conducting a systematic Best Possible Medication History (BPMH) interview is an integral part of Med Rec, verifying the information with another reliable source, ensuring complete documentation of the drug name, dose, route, and frequency to create a complete snapshot of a patient’s medications.
The organization required three modules as a part of the Med Rec roll out:
- Module 1: An Introduction to Med Rec
- Module 2: Conducting a Best Possible Medication History
- Module 3: Entering the BPMH into the Electronic Patient Record (EPR)
The demo will focus on Module 2.
Articulate Storyline and GoAnimate.
Approximately 1,500 RNs and about 1,000 allied healthcare workers.
Two weeks for Module 2 (which I will be demoing), two months for the entire project consisting of Modules 1 – 3.
How do I model a specific behavior in an online learning environment?
Modelling behaviors that you would like to see is very important and, though it can be challenging, it is not impossible.
